Pauls Valley, Okla. — As the school year approaches, the Garvin County Health Department encourages families to start thinking about their students’ backto- school checklist.
The back-to-school checklist consists of scheduling outstanding immunizations, obtaining shot records and ordering birth certificates.
To accommodate families seeking immunizations, the Garvin County Health Department is offering immunization clinics on July 30 and 31, and August 6. Appointments are available from 1 to 4 p.m. To schedule an appointment, call 405238-7346. Immunizations may be available on other days; however, parents and guardians are asked to call ahead.
Immunization records can be accessed through the Oklahoma State Immunization Information System (OSIIS) portal, available at shotrecords. health.ok.gov. To retrieve records, parents must provide the child’s full name, date of birth, and the contact phone number or email listed on the child’s immunization record. The Garvin County Health Department may be able to assist with accessing records.
Birth certificates can be ordered online or by calling 877-817-7364, or visiting brnw.ch/VitalChek.
Requests made by a parent listed on the record are typically processed within two business days and arrive by mail within a week. The health department encourages ordering birth certificates before August for timely delivery. To avoid delays, all applications should be filled out completely and accurately.
